OR Nurse Handoffs

I am looking for information regarding how OR nurses in other facilities do their handoffs/reports to PACU/SDS/Special Care Units after surgery. With the increased emphasis on rapid turnovers, many nurses feel pressured between cleaning the room and giving a report. At our facility the patient is accompanied by anesthesia and a resident after surgery. Initial assessments recorded in the electronic record by the Same Day unit are not readily available to the OR nurse, and the same is true for the recovery nurse accessing the OR nurse's documentation.
I'm considering a paper form that starts in SDS, is added to in the OR, and is then given to the recovery nurse. I would be very open to hearing what is working well for others and/or in obtaining a copy of any forms in use.

We use a paper handoff form exactly as you describe, it is initiated in the preop area (either outpatient Same Day Surgery or Inpatient Holding area) with some basic patient information, added to by the OR nurse, and follows the patient to PACU.  One thing I think is helpful is that we included a body diagram, so rather than writing longhand we are able to sketch, circle, or shade in areas to indicate incision sites, skin conditions, device locations, grounding pad site etc.  Very quick way of relaying this type of info.  The form also includes stuff that doesn't need to go into the patient record, like where the family is waiting, and what personal items came to the OR with the patient. 
Our process is to use the form to guide verbal handoffs.  We worked to include all relevant info that we expect in perioperative handoff, in a logical order that makes sense to both parties.

would you be willing to share a copy of your handoff tool with the diagram. It sounds very helpful.
Are you using an electronic documentation system? If so what portion of the handoff is documented in your EMR?

We are currently doing a patient satisfaction intiative and have a QI OPS to OR handoff initiative which has been in place since July 5, 2017.  I would be happy to send you a copy along with our PACU handoff which we have been using for years.  The plan is to revamp these two handoffs and incorporate the debriefing into a single sheet which will be great for doing audits.  I will be happy to send you what we have just know we are in the process of revamping this entire handoff process.

What a great feed.  Thank you for sharing your expertise.  I wanted to also let you know that AORN has a member-only tool kit with many resources on the Hand off/over process.  Once you are logged into the AORN website it is under the Guidelines & Clinical Resources header, then tool kits.

Hello Everyone,

About a year and a half ago we created a paper form to assist with communication/hand-offs between the different perioperative areas.  We were asked to put it on hold so that a larger group could start to meet and look at ways we could use the Electronic Medical Record.  We did decide on a process using the "Hand-off" Adhoc form in the EMR.  Each area (SPAR, POHA, OR, PACU) is to use the hand-off form to communicate to the next area of care.  It is certainly not perfect but it is a part of the medical record and wherever the patient goes all of the nursing staff are able to view what has been going on with the patient.  I am not sure if any of you have the same capability, but if so I would suggest you take a look and even trial it.

For our inpatients, we use a paper form, but also enter it into the Electronic Medical Record. We are a critical access hospital, and do not have computers in every patient room, so we feel like for safety we need to give this written form at hand off report and assessment time. PACU is staffed by my OR crew, and the circulators often times start the OR case, recover, then take the patient to their home unit.  We also have a tool for OR to PACU on the occassion that I have a separate PACU, nurse that I did attach, but most of the time we use the Inpatient tool.  I'm sure it isn't detailed enough for larger facilities, but It works well for the cases we do here.
